What to expect at your appointment

Our intake coordinator will make an appointment with you for the therapist that meets your needs by schedule, specialty and payment type.  If you are using health insurance you will need to pre-authorize with your insurance company prior to coming in for your appointment.  You will brief forms to fill out on your first visit.  Co-payments or self pay payments are due at the time of service.  Please provide 24 hours cancellation notice, if you are unable to make it to your appointment.
